POSITION: Family Stability Case Manager

PROGRAM: Indian Child Welfare Program

SALARY: $42,000-$46,000

TYPE: Full-Time M-F 8-4 pm Non-Exempt

 

Program Description:

The Family Stability Case Manager position works with American Indian families with children under the age of 18, who live in Hennepin County and are in the early stages of family instability. The position emphasizes advocacy for children and families and works to establish supportive relationships and empowers families to build on their strengths.

 

Qualifications:

  • Prefer bachelor’s degree in human services or related field. Will consider multiple years of experience in social services if degree is not completed or AA degree with years of experience.
  • Knowledge of Child Development, addiction, and experience working with American Indian families.
  • Knowledge of the Child Protection System is a plus.
  • Possess excellent communication and written skills.
  • Possess awareness of the American Indian community and its needs.
  • Knowledge of the Data Privacy Policies for social services agencies.
  • Native American preference.
  • Knowledge of community resources and recovery support services.
  • Willingness to assist in providing holistic and cultural services to clients.

 

Duties and Responsibilities:

  • Conduct intake with new clients to assess needs and short-term goals.
  • Refer clients for chemical health programs, Rule 25 assessments, treatment centers, methadone or other programs, and recovery support services.
  • Refer clients for cultural, social, educational, and employment services.
  • Provide client transportation to appointments as needed.
  • Work collaboratively as a member of the ICWA staff team.
  • Provide intensive case management with each individual client, making contact 2-3 times weekly.
  • Conduct home visits; Participate in occasional supervised visits between the parents and children.
  • Maintain client confidentiality.
  • Provide cultural services to clients and learn about holistic and cultural teachings.
  • Assist in conducting program evaluation forms, groups, etc. along with Program Evaluator.
  • Maintain and keep timely records, and correspondence with service providers when requested.
  • Maintain statistical data and provide semi-annual reports to Program Director.
  • Attend monthly Minneapolis American Indian Center staff meetings, ICWA Program meetings, and community events as it pertains to social service work – as necessary.
  • Various duties as assigned by Program Director.
  • Must have no drug/alcohol/substance abuse related problems.
  • Reliable transportation required with valid Minnesota driver’s license and proof of current insurance.
  • Must complete background check.
